  • Your child's name and date of birth
  • Your insurance, military or Medicaid card
  • Your preferred office and days
  • Anything that helps your child feel comfortable (see your comfort plan above)

Insurance we accept

Military and most commercial plans, plus Virginia Medicaid and FAMIS (Cardinal Care Smiles). Our Red Mill, Chesapeake, Portsmouth and Town Center offices also accept North Carolina Medicaid.
